Participation & Consent Form

Parent/Guardian Agreement

I, the undersigned, have read and fully understand the rules and regulations that govern my child as a participant in athletics at MICAH Christian School. I understand that athletics are an extra‑curricular activity and that attendance at all practices, games, and special functions is required for my student.

I understand that, at times, fundraising efforts may require me to provide items for my student to bring to school or to participate in events/fundraisers. I agree to support these efforts when asked. I also agree to volunteer when needed to assist with athletic events, including working concessions, door admissions, or other assigned duties.

I give consent and authorize school officials to obtain, through a physician of their choice, any medical attention deemed reasonably necessary for the welfare of my child if they become injured or ill during school activities. By signing this form, I release MICAH Christian School, Maryville Baptist Church, and their representatives from any and all liability.

I give my consent for the student named below to represent MICAH Christian School in extra‑curricular activities. I also give permission for them to travel with the team to other schools or events by Maryville Baptist Church/MICAH Christian School bus or van transportation, or by a volunteer driver in a personal vehicle.

Student Section

I have been informed and understand the rules governing participation in athletics at MICAH. I agree to abide by all rules listed above. I understand that failure to follow these rules may result in disciplinary action or dismissal from the team.
